Nystagmus and Related Ocular Motility Disorders

چکیده
General Concepts and Clinical Approach This chapter concerns abnormal eye movements that disrupt steady fixation and thereby degrade vision. We now know a good deal about the normal anatomy, physiology, and pharmacology of ocular motor control (1). Our approach is to apply this knowledge to nystagmus and other ocular oscillations, since pathophysiology provides a sounder conceptual framework than a system based solely on phenomenology. We first summarize the mechanisms by which gaze is normally held steady to achieve clear and stable vision (2). We then discuss the pathogenesis and clinical features of each of the disorders that disrupt steady gaze, including the various forms of pathologic nystagmus and saccadic intrusions. Finally, we summarize currently available treatments for these abnormal eye movements and their visual consequences.

Ocular motility and Wilson's disease: a study on 34 patients.
BACKGROUND Wilson's disease is an autosomal recessive genetic disorder resulting from an abnormality of copper metabolism. The excessive accumulation of copper in the brain induces an extrapyramidal syndrome. Oculomotor abnormalities occur in most extrapyramidal disorders but have rarely been studied in Wilson's disease. OBJECTIVE To evaluate the ocular motility manifestations of Wilson's dis...
متن کاملLatent nystagmus and acquired pendular nystagmus masquerading as spasmus nutans.
SUMMARY We used ocular motility recordings to identify the characteristics of a rare combination of conjugate, horizontal jerk, and pendular nystagmus in a 9-year-old boy. The clinical diagnoses were amblyopia, left esotropia, congenital nystagmus, and an apparently uniocular pendular nystagmus that mimicked spasmus nutans. Ocular motility recordings revealed an unusual latent/manifest latent n...
متن کاملAcquired Ocular Motility Disorders and Nystagmus
Eye movement disorders may be considered in two categories: those that cause incomplete eye movements (ophthalmoparesis) and those that cause excessive eye movements (saccadic intrusions and nystagmus). Central to an understanding and correct diagnosis of abnormal eye movements is the evaluation of ocular alignment, ocular motility, and each functional class of eye movements: optokinetic, vesti...
متن کاملLithium-Induced Downbeat Nystagmus and Horizontal Gaze Palsy
We report a case of lithium-induced downbeat nystagmus and horizontal gaze palsy in a 62-year-old woman who was treated for a bipolar affective disorder with lithium carbonate for one month. At presentation serum lithium was within therapeutic range. No alternative causes of the ocular motility disturbances were found, and the patient improved significantly as lithium carbonate was discontinued.
